Community Nurses make a difference everyday to the lives of the people they visit at home. We are proud of our Community Nurses who provide increasingly complex care for patients and support for family members to encourage patient's to receive the care they need in their own home., As a Community nurse, you'll

  • assess the healthcare needs of patients and families
  • monitor the quality of care they're receiving
  • be professionally accountable for the delivery of care
  • utilise technology to enable you to update records as you go
  • Your patients could be any age, but they'll often be elderly, while others may have been recently discharged from hospital, be terminally ill or have physical disabilities. The service aims to provide an excellent standard of care that supports patients to remain in their home environment. In Northamptonshire you will work as part of a multi-disciplinary team in order to support the service and focus on delivering quality outcomes and improving the patient experience. You'll play a vital role in keeping hospital admissions and re-admissions to a minimum and ensuring that patients can return to their own homes as soon as possible.

    We are looking for experienced Registered Nurses with a passion for high quality nursing care and also experience in either acute medicine, Emergency Care or community nursing. The post holder will provide acute nursing assessment & interventions in patients' own homes and will ensure that patients are referred onwards as appropriate to community services once their care episode is complete., You will need to be adaptable and resourceful. You'll need to be well organised, confident and able to cope in diverse situations to deliver patient care. Leadership and management skills are also important, as with all nursing roles, communication skills,problem solving and good judgement are crucial. You will be part of caring team and be able to work flexibly with minimum supervision, with the ability to prioritise work based on the patient need., (Please be advised that this position is subject to the Health and Social Care Act 2008 (Regulated Activities) (Amendment) (Coronavirus) Regulations 2021. All applicants must be eligible to work in a CQC registered Care Home and therefore must confirm that they have had a full course of an MHRA approved Covid-19 vaccination, unless exempt. Any successful applicants will be required to evidence their vaccination status as part of their pre-employment checks.)
