Full Job Description

As part of a team you will be responsible for the assessment of nursing care needs, implementation and evaluation of care plans and the delivery of high quality holistic care within the community setting.
You will need to be able to work independently, possess good time management and organisational skills and be expected to be innovative and flexible to work in other areas if required to meet service need. The work is varied and interesting and will require knowledge and experience in the management of palliative care patients. Travel is an essential part of this role; please demonstrate on your application form how you would meet this requirement.
The post holder will participate in the delivery of nursing care and support to patients in their own homes and have the necessary confidence and competence to work autonomously.
Working under the direction of the Team leader, being responsible for assessment and management of the health needs of an identified, this includes end of life care, wound care, and other clinical interventions and/or complex nursing needs.
