Full Job Description
We are offering an exciting opportunity to join our District Nursing Team within Lichfield and Burntwood PCN as a Band 5 Community Staff Nurse., As part of this role you will have responsibility for a defined caseload of patients to provide care directly to patients on the caseload with a range of nursing needs (including palliative care, complex wound care, pelvic health, medication administration). Working in collaboration with the multi-disciplinary team and colleagues to promote independence and support patient centred care planning. You will be expected to:
- Deliver care to patients who are in their own homes or residential homes, and or delivery of care to patients within a clinic setting where these are available and appropriate.
- Be responsible for Patients in a variety of settings across the Trust including the community and hospitals.
- Be skilled in the assessment of care/support needs and in the development, implementation and evaluation of care/support packages.
- Work autonomously, in line with your scope of practice, competence and policies.
- Handover patient information to the wider team, document patient information effectively.
- Escalation of patients' needs as appropriate.
We are looking for a passionate Community Staff Nurse with demonstrable advanced clinical skills who is innovative, motivated and enthusiastic with excellent communication and organisational skills. You will have a commitment to providing holistic care to patients in their own homes, be able to work on your own initiative and work well within a team. Key clinical skills include both simple and complex wound care, including management of various types of drainage systems, bladder and bowel management, palliative and end of life care as well as medication administration. Within the community we are seeing more complex and acute patients than ever before, with increasingly specialist treatment and care being provided by highly skilled staff. Working within our Community Nursing Team you will support, assess and treat patients with a variety of health needs, delivering high quality care in line with evidence based practice. This will be supported with ability to complete robust record keeping. Due to the nature of community working, ability to travel across the patch to provide care to patients is essential. - Supporting your career development and progression
