Full Job Description
An exciting opportunity has arisen for experienced qualified Nurse to join the Gosport, Integrated Care Team. Working in the community you will be providing care to patients with complex care needs, enabling patients to receive the care they require to remain in their own home., Working within Professional guidelines and accountability, the post-holder will have responsibility for the management of a defined community caseload (via Nursing, Physiotherapy or Occupational Therapy interventions) for a group of GP Practices as part of the Integrated Care Team.
The post holder will have responsibility for managing co-ordinating and delegating work ensuring referrals are dealt with according to clinical need, Holistic assessments and care plans are recorded in line with Trust and Professional record keeping standards and clinical care at all times meets acceptable standards of patient safety and quality outcomes.
The post holder will be required to work with a high degree of autonomy and professional accountability. A primary aim of the role is to prevent unnecessary admission of patients to an acute hospital and support timely discharge into the community. The post holder will also have a responsibility for re-enablement, rehabilitation, health protection and promotion programmes that improve and reduce inequalities.
We are looking for an enthusiastic, caring & motivated professionals with excellent clinical, organisational & communication skills, who have the ability to work autonomously.
The post holders must have relevant experience in health care and demonstrate a proactive and positive attitude to learning the skills required to fulfill the role.
Access to a vehicle and possession of a full driving license is essential.
