Full Job Description
We are looking for a dynamic, enthusiastic and motivated individual to join our workforce in a busy and well established community team. Greater Wealden PCN provides nursing services to Heathfield, Uckfield and surrounding area. Our community nurses benefit from having continual opportunities to develop an excellent skillset and become autonomous practitioners whilst working as part of a team delivering high quality, holistic care to patients in their own homes., a) Holistic assessment and care planning for patients in their own homes..b) Individualised treatment and the promotion of self-care. c) Admission avoidance and enabling patients to remain at home where possible c) Cross service collaboration and referral to appropriate members of the multidisciplinary team.d) Development of extensive nursing skills with excellent training opportunitiese) Professional development as required to maintain area of practice, asnegotiated in supervision.f) An awareness of health, safety and security risksg) Engagement in service provision and change.h) Acts as a practice supervisor for staff and students.
