The position is primarily based with the Hailsham & Eastbourne Teams, but flexibility to travel to other locations as needed to support service requirements is essential. It can be quite a challenging role at times, so a positive attitude, compassion, resilience, and vigilance are important. It is essential that that you have access to a car, and hold a full licence. We provide a seven day service to patients from 08.00-20.00 hours (including weekend working & Bank Holidays)., What you will be doing
+ Assessing new patients for nursing interventions in their own homes, creating & evaluating care plans
+ Undertaking skilled nursing interventions for a devolved caseload with support of the District Nurse & Clinical Leads
+ Assisting patients & carers in achieving optimal independence using best practice & following national & Trust policies & guidelines

You must be able to travel independently, carrying equipment & car sharing as required. You must have access to a car and hold a current full driving license.
