Full Job Description

Within the Community Services Division of Wrightington Wigan and Leigh NHS Foundation Trust , The Frailty SDEC supports individuals who require both short term medical interventions and a period of bed-based, Nurse & Therapy led rehabilitation. It will support individuals to achieve specific goals which enable safe discharge back into the community or their own home., Under the supervision of the ward manager the nurse is responsible for the provision, organisation and direction of nursing care of the patients in his/her care. To communicate effectively with all members of the multidisciplinary team to ensure safe and high quality care is provided to all patients.

Frailty SDEC is operational 24/7/365 so shifts will be day or night. This is a full time role (37.5 hours) on a 12 month fixed term basis to cover maternity leave. If the substantive postholder does not return, it may be made permanent.
