Full Job Description

Come to work in a Children's Service rated outstanding by CQC and which is Baby Friendly Accredited. We provide care to a diverse community and work closely with the Family Hubs in Derby., You will be responsible for the delivery of health visiting services to a defined corporate caseload within Derby City. You will also be expected to work within the 0-19 framework ensuring a seamless service to the family as appropriate.

You will lead on the delivery of the Healthy Child Programme as defined within the service specification, identifying packages of care based on the Universal, Universal plus, Universal Partnership plus models of service delivery.

You will provide leadership and supervision to other members of the Public Health Team ensuring you delegate appropriately to other team members and work within Derbyshire Healthcare NHS Foundation Trust policies and procedures.

There will be an expectation for you to work collaboratively with other agencies and partners, ensuring a seamless delivery of services.

From supporting families during the antenatal period whilst delivering the healthy child programme, you will be a skilled professional with excellent communication skills who makes a real difference to improve the health outcomes for children and their families in Derby City.
