Full Job Description

The post offers varied and interesting community experience in a well supported environment and is an ideal opportunity for a nurse interested in working in the community. Applicants will need to demonstrate the ability to assess, plan, provide and evaluate evidence-based nursing care for a wide range of conditions. The team covers a 7 days a week service and therefore a flexible approach to working is required., Providing a high standard of patient care using evidence based framework. The post holder is a member of the Integrated Community Nursing Team and may take responsibility in the absence of the Community Nursing Sister for the team and caseload.
The post holder is responsible for assessment, planning, implementation and evaluation of individualised nursing care to patients living in the community. The service is provided 7 days per
