Full Job Description

An exciting opportunity has arisen for highly motivated individual to join our Community Nursing Team covering the North East locality of in the provision of planned care to patients in the community. We are looking for a caring, motivated and friendly Community Nurse to join our team. You should possess commitment to interdisciplinary team working and innovative delivery of excellent standards of care. Main duties of the job You will be able to demonstrate a real enthusiasm for community nursing, as well as a desire for on-going professional development. We also actively encourage newly qualified nurses to join our team. In return we offer a preceptorship programme and support professional development in a nurturing environment, allowing you to reach your full potential. The team provides nursing care to patients within their own homes or chosen place of residence offering an alternative to hospital admission, supporting timely discharge and enabling people to realise their potential. We are committed to clinical support, CPD & training. The post is covering 7 days, 8am to 6pm service.

Essential

  • RGN NMC Registered.
  • Ability to demonstrate a level of English language and grammar commensurate with writing in service user case notes.
  • Experience Essential
  • Post registration general experience.
  • Experience of working in a multi disciplinary / multi agency health / social care environment.
  • Experience and knowledge of basic administration systems including Microsoft office.
  • Experience of successful partnership working arrangements with other services.
  • Previous community experience.
  • Skills Essential
  • I T skills or willingness to undertake RIO Training.
  • Up to date with Mandatory Training.
  • Uses evidence based practice.
  • Works within own defined competencies.
  • Awareness of changing healthcare factors influencing change.
  • Well refined communication (verbal and written), interpersonal skills - ability to gain rapport at all levels.
  • Engaging and empowering approach that seeks to work with others and deliver outcomes in a successful and well informed manner.
  • Demonstrates tact and diplomacy.
