Full Job Description

The post holder will work within Hounslow School Nursing team to promote and support the health and wellbeing of all school-aged children, their families and the wider community of Hounslow.

The post holder will be expected to carry out assessments of needs with school-aged children, young people and their families. The post holder will also work on the implementation of planned programmes of care delegated by the School Nurse specialist practitioner and public health activities., CLINICAL RESPONSIBILITIESo Contribute to the identification and protection of children at risk of abuse and act in accordance with the local Safeguarding Children's Board guidelines. To attend safeguarding meeting and other meetings to improve the health outcomes of children and young people as directed by the School Nurse Specialist Practitioner and provide written reports as required., o Support delivery of the 5 -19 Healthy Child Programme (DoH 2009) including the National Childhood Measurement programme (NCMP) vision screening programme and health and wellbeing questionnaires under the direction of the School Nurse Specialist practitioner., o work in co-operation with all staff within the Trust to utilise resources effectively, o Promote and respect equality, diversity and the rights of the individuals, families and colleagues., To have awareness and participate in clinical audit and research. To participate in the search for health needs and the creation of public health plans and key health priorities for schools. To support schools to meet any unmet health needs utilising all the services provided and working in partnership with the local authority directed by the School Nurse Specialist Practitioner
