This role requires day-to-day delivery of patient care in our Integrated Community Stroke Multidisciplinary Team of AHP's, together you all make the difference. You will need to:
To use specialist clinical reasoning, critical thinking, reflection and analysis to support assessments, treatment and management of stroke survivors with complex presentations. To work closely with the MDT, communicate effectively with clients/carers and their families and supervise, teach and develop other staff as required.
Support our patients within the community to enable them to live their optimum life, being self-caring and independent with their health needs following a stroke.
Demonstrate excellent communication and interpersonal skills with the ability to be adaptable, reliable, resourceful and approachable, evidencing high levels of personal and clinical credibility, being able to establish effective working relationships in a range of work settings., We are looking for a part time Physiotherapist to join our Integrated Community Stroke Team across Nottingham South. We are looking for someone is able to demonstrate motivation, drive commitment and passion for promoting health, well-being and independence for stroke patients in the community. This post will allow the successful applicant the opportunity to work collaboratively within an integrated multi-disciplinary team, providing specialist interventions to patients referred to the Community Stroke Rehabilitation services, including early support stroke rehabilitation. The multidisciplinary team includes Occupational Therapy, Speech and Language Therapy, Physiotherapy, Clinical Neuro-Psychology, Assistant Practitioners and Rehabilitation support workers.
The service enables the accelerated discharge of stroke patients into their home environment, providing specialist rehabilitation in the community. You will have a key role in helping facilitate and support timely discharges from in patient setting such as the acute hospital by working closely and providing in reach joint working with our close partners. The team provides on-going rehabilitation in a variety of community settings focusing on timed specific patient goals and also embraces the needs and ability of their carers. You will have a key role in the management of complex patients throughout the pathway.
We are therefore looking for an enthusiastic and motivated highly specialist Physiotherapist who is able to manage complex cases and situations and able to adapt to working in different environments.
You must have a
- Recognised Physiotherapy Degree Qualification
- Health Professions Council - Licence to Practice
As part of the community stroke therapy service, the post-holder will be required to have a valid full UK driving license and have access to vehicle for work purposes; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
The service runs from 8am - 5pm during Monday to Friday, the service aims to provide weekend therapy input with bank holiday working also required.
BASE: The Community Stroke Team cover the South of the County which includes three potential bases: Stapleford Care Centre, Park House and Lings Bar Hospital. This position will be based in the Nottingham West locality based at Stapleford Care Centre.

This post is eligible for a Golden Hello payment of £2,750 for new starters who are external to the Trust. This is payable in 3 instalments over a 2 year period. This payment is only eligible to new starters who do not already hold a contract of employment with the Trust and have not been an employee of the Trust in the previous 3 years. This post also includes paid enhancements for unsocial hours working during night, weekends and bank holidays.

Care is currently delivered 8am-5pm, 5 days a week, including Bank Holidays. Weekend working may be required in the future.
