Place Based Team - Warwick & Kenilworth strives to provide patient centred care, and actively encourage prevention and self-management, therefore the applicant will be required to work closely with the multi-disciplinary team, working collaboratively, and strengthening relationships with our GP's., The Band 3 Community Support Worker will undertake various differing clinical skills, such as complex wound care, as well as care of chronic lower limb disorders and application of specialist compression therapy, under the direction but not direct supervision of the registered staff and working as part of the multi-disciplinary team.

As a Community Support Worker (CSW) you make a valuable and important contribution to the delivery of high quality healthcare, supporting the registered professional in the delivery of safe, effective, compassionate patient care. This job description is to be read in conjunction with the person specification for this particular job.

As a Clinical Support Worker you will need to be flexible and hardworking as you will be expected to see patients in their own home environment, under minimal supervision within your competencies and adhering to SWFT's Lone Working Policy.

You will have at least 1 year care experience preferably in the community and have some knowledge and skills in managing the care of long term conditions, wound and pressure ulcer care and end of life patients. Enthusiasm and excellent communication skills together with being a good team player are all attributes we are looking for.
