Join Our Team as a Community Support Worker! We're looking for a dedicated and empathetic individual to join our Prevention Assessment Team (PAT) as a Community Support Worker. This vital role is part of a multi-disciplinary team led by West Sussex County Council (WSCC) and works in close collaboration with local partners to support residents who need guidance but don't meet the thresholds for statutory services., As a Community Support Worker, you'll be providing crucial information, advice, and support to individuals with identifiable needs, helping them maintain their independence and promoting their health and wellbeing. You'll play a key role in preventing crises by connecting people with the right services at the right time.,

  • Promote Health & Independence: Conduct wellbeing conversations and encourage self-help through a person-centred approach.
  • Connect People with Services: Support individuals in accessing necessary health, social care, and community services. Complete relevant assessments and paperwork.
  • Coordinate Care: Manage cases within the voluntary sector and work collaboratively with community services to provide Community Led Support (CLS).
  • Prevent Crises: Act as a liaison between people at risk and health and social care professionals, taking proactive steps to prevent deterioration or crises.
