Full Job Description

As a Community Support Worker in an integrated team with the West Essex Care Coordination Centre, you will be undertaking assessments and reviews in line with our statutory duties under the Care Act to identify eligible needs and to then generate a person-centred and outcome focused support plan. You will be working in partnership and developing relationships with both internal and external colleagues / partners to achieve the best outcomes for Adults and Carers. This is a "community-based" role, so you can expect to be visiting adult in their home and the community hospital. You will be meeting colleagues and working in the office as well as working from home. The role covers 7 days per week and Bank Holidays on a rota basis. You can expect a varied role, supporting adults of all ages (18+). Working as part of a supportive and knowledgeable team that encourages your career development and growth. You will demonstrate a commitment to learning and development to support yourself and the team to deliver the best service possible. An induction and appropriate training will be provided.

An essential attribute for the role is to be 'curious' as thinking 'outside the box' is a must to achieve the best outcomes and to fully engage and utilise all that our community has to offer. You'll bring a strong ability to build effective relationships with adults and their families in times of challenge and crisis. You will have the drive and passion to want to make a difference, and a willingness to learn. Ideally you will have some experience of activity-based work with adults and some knowledge of legislative and policy guidance applicable within social care settings.

As part of employment with Essex County Council (ECC) should your role require a DBS check you will be required to register with the DBS update service. The DBS update service ensures all registered DBS certificates are kept up to date and allows authorised parties to view the status of your certificate.
