An exciting opportunity has arisen for a Community Support Worker to join our well established Intensive Community Assessment & Treatment Team based in Nottinghamshire. We are looking for someone with enthusiasm and energy who wants to make a difference to people's lives; is creative, innovative and highly motivated.
ICATT provides a rapid, short term assessment and treatment service for people with an Intellectual Disability who challenge services and may have additional mental health needs. The Team seek to provide an alternative to hospital admission where it is appropriate to do so, enabling service users to receive treatment and care within their own homes, remain involved in their care as part of their own community and therefore minimising the disruption of their lives and the lives of those caring for them.
The post would suit someone who has a sound knowledge of and experience in working with people with an Intellectual Disability . You will be required to work with a delegated caseload and carry out relevant physical and mental health observations and interventions, to undertake nursing, therapy and social activities, support and monitoring of service users living in the community.
You will participate fully as a multidisciplinary team member working in partnership with social care and voluntary/independent sector providers. You will provide holistic advice and health education to service users, their carers and community support agencies., You will need to have excellent communication skills, be able to exercise clinical judgement in respect of assessing and managing risk. In addition, you need to be able to help deliver a safe, timely, caring and responsive service. You must have assessment and care planning skills and experience in multi-disciplinary working and supporting people with an Intellectual Disability and or Autism.
You will have responsibility for facilitating time limited therapeutic treatments and care for individuals on your delegated caseload. You will be expected to record and report clinical observations when supporting individuals and assisting in their treatment plan in the community, noting any physical and mental health changes in a timely manner.
You will be expected to liaise closely with other health and social care professionals as well as with patients' family and carers. You will need to be resilient and prepared to think about the positives rather than focusing on the challenges.
