Full Job Description
As a Community Support Worker in the Community Options Team, you'll be at the heart of a mission to enhance the lives of adults with disabilities who rely on our services. You will play a hands-on role in creating tailored support plans that align with each person's unique needs, talents, and aspirations. You'll be supporting adults to rebuild confidence, develop essential life skills, and reconnect with their community through leisure activities, education, and independent living. No two days are ever the same, as you'll be working in a variety of settings-whether it's helping someone gain work experience, encouraging an individual to try a new hobby, or providing personal care as outlined in their Care and Support Plan.
Your role will also involve building strong, trusting relationships with families, carers, and professionals from other agencies to create a truly collaborative approach. You'll be the person others look to for insight and guidance at review meetings, where you'll provide detailed, objective feedback to help shape the future care of the people you support. We know that great support is built on creativity, flexibility, and an eye for opportunity, which is why we're seeking someone who can think outside the box and spot new ways to encourage and inspire those around them.
