An opportunity has arisen for Community Mental Health Support Workers within the Community Mental Health Teams (CMHT) West of Northamptonshire based at Campbell House, Northampton.
We are looking for Community Mental Health Support Workers with demonstrable skills, enthusiasm and compassion to work with service users who have a range of complex mental health needs who is focussed on supporting service users to recover. You must have experience of working within a healthcare setting.
Community Mental Health Teams aim to provide planned and effective mental health treatment alongside the service user, their carers and other services in line with the goals of the service user. The CMHT ensures service users needs are met by using mental health support services across the breadth of the community. You will be a key person to ensure this is delivered by working collaboratively to achieve the best outcomes for service users.
The Community Mental Health Support Worker will contribute to the ongoing assessment, planning, implementation, and evaluation of care provided to service users. The Multi-disciplinary Team holds overall responsibility for the care provided with each service user, with you being the identified keyworker for a cohort of service users in the team who is the point of contact for them and their carer., The CMHT team consists of a wide range of professions including Nurses, Occupational Therapists, Support Workers, Peer Support Workers, Psychologists, Psychiatrists and Wellbeing Navigators.
You will work closely with the wider Community Mental Health Teams to provide outstanding care for service users under the community service.
You will keywork a cohort of service users in the team and will be required to monitor their mental state whilst delivering a range of recovery-focussed interventions.
