Full Job Description

We have a fantastic opportunity for a Community Support Worker to join our Community Behaviour Support Service (CBSS) in the North East! This is a part-time role, contracted at 22.5 hours per week. Due to the travel requirements of this role, it is essential that you are a driver with access to your own car. As a key member of the Community Behaviour Support Service, you will be working in the community with families and nursing home staff caring for people with dementia. By giving practical advice and handson support, our team aims to reduce the impact of these challenges on caregivers and, where possible, prevent the challenges from happening in the first place. The role will involve some aspects of lone working with support and guidance from qualified staff. You must therefore be capable of independent travel.,

  • Support qualified staff in initial contact with new clients.
  • Support qualified staff in the assessment of needs of individual clients.
  • Assist in developing treatment plans and strategies for dealing with individuals challenging behaviour.
  • Form a strong relationship with both clients and their carers built on mutual support and respect.
  • Work in partnership to implement planned written support.
  • Provide written/verbal communication for other staff. Ensuring all nursing notes are accurate and up to date.
  • Demonstrate good practice and work in partnership with carers and families.
  • Act as a liaison for families and carers, offering advice and support.
  • Respect and maintain confidentiality.
  • Liaise with external agencies, ie, social workers, CPNs, GPs, 30 days annual leave plus bank holidays (rising to 32 days at 5 years service) and the option to purchase or sell days
