Full Job Description

An exciting opportunity has arisen to recruit a Band 3 Community Support Worker to our Memory Team in Scarborough. The post holder will be part of a passionate and innovative multi-disciplinary team, exploring new ways of working and contributing to the effective provision of high-quality assessment and diagnostic pathways for older adults with concerns about their memory. The post holder will provide a range of services including keeping in touch calls with service users and families prior to first contact with the team, support with physical health checks, both prior to and after prescribing where appropriate, practical and emotional support to individual service users, their carers and families as indicated by the individual care plans co-created by the person and their Link Worker., We are looking for a dynamic and motivated individual with a passion for older adult mental health care and who will work with the Memory Team, to deliver a high standard of care and support. You will be joining a committed and supportive group of clinicians who really do put patients at the centre of what they do. You will be working alongside a multi-disciplinary team of practitioners, including Nurses, Psychiatrists, and Psychologists. The job will involve supporting professionally qualified staff, often without direct supervision, to implement, monitor and evaluate person-centred, co-created plans of care; ensuring that service users and care givers within their designated caseload receive appropriate care and support depending on their needs. Previous experience of working with older adults in a health/social care setting is essential and experience of supporting older adults with physical and/or mental health problems in the community would be preferable. The successful candidate must have completed the Diploma in Health and Social Care Level 2 or equivalent and have a Care Certificate to be considered for this position. Candidates must also have Literacy and Numeracy level 2 or equivalent and must complete the Trust Healthcare Assistant competencies within their probationary period (first 6 months in role)., independently, is essential for this post.
