Full Job Description
The post holder will work as a member of the Community Therapy & Rehabilitation Team to provide assessment and rehabilitation as a part of the wider multi-disciplinary service.
The post holder will provide assessment and treatment to patients with diverse health needs within the community setting, in peoples own homes, but with flexibility to work in other areas as required.
You will assist qualified Occupational Therapy and Physiotherapy staff in the education and training of staff from own and other disciplines within the multi-disciplinary service, including training to care and other support staff.
To provide feedback to the qualified Occupational Therapist/Physiotherapist and recognise own limitations
The post holder will engage in regular informal and formal supervision.
To assess, provide and check the provision of aids, equipment and exercises to patients on your defined case load.
Be responsible for managing a delegated caseload on a day to day basis including prioritisation of tasks.
Work in the community as a lone worker in line with Trust and departmental Lone Working Policy.
Adhere to the code of ethics and conduct for Occupational Therapy and Physiotherapy and Allied Health Professional standards of practice, as required by the team.
Working for our organisation
We operate from three main hospitals-Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community healthcare premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.
FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.
WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses.
All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, day case and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.
The therapy team consists of Occupational Therapists, Physiotherapists, Technical Instructors, Therapy Assistant and Admin Support.
Applicants must have a genuine interest in people and be enthusiastic and motivated in enabling people to regain and retain their independence.
The service works closely with the Integrated Care Communities, local GP's, Adult Social Care, Advanced Clinical Practitioners and hospital discharge teams.
Applicants must have the ability to travel to provide assessment and treatment to patients in the community and attend meetings.

The service supports people with a wide variety of conditions including elderly care, frailty and falls, orthopaedics and long-term condition management.
