Full Job Description

As a Community/Treatment Room Nurse you will deliver safe, clinical decision-making and expert care for patients within the Trust. You will be expected to maintain general nursing duties including undertaking wound care & removal of sutures, along with collaborative working alongside GPs, community nursing specialists and hospital specialists., To holistically assess care needs, plan, implement and evaluate packages of care for patients based on best practice and evidence.

Respond to circumstances which require immediate action or response outside of the identified care plan.

Develops and maintains complex clinical skills to carry out clinical procedures e.g. syringe drivers, complex wound care, suprapubic catheter, injections.

Communicates complicated and sensitive information to patients, families and carers, promoting understanding whilst recognising barriers to communication that may exist.

Assist in health promotion / education of patients and their families and carers

Assist patients and their families / carers in achieving optimum health status through empowerment. Provide advocacy for patients and their carers, recognising ethical dilemmas in care.

Ability to use own initiative.

Contribute to immunisation programmes, Implements patient group directions

Share and cascade knowledge to other team members, acting as a link nurse within a specialist role.

To work within a clinic or home environment providing holistic care to patients including wound care, suture removal, catheter care, venepuncture, medication administration and support., LCHS recognises the increasing demand for more flexibility in working practices in order to accommodate the personal goals and commitments that employees experience at different stages during their working lives. A basic knowledge of computers is essential. Previous use of SystmOne would be an advantage however full training will be given.

An enhanced DBS check will be required for this position.

The Post is 0.4 WTE within a set clinic based in long sutton, and the remainder is covering community and treatment room across the county. The Applicant will be expected to travel to to clinics such as Boston, Spalding, Skegness, Spilsby and Mablethorpe.
