Full Job Description
We are recruiting a Band 5 Community Triage Clinician post in the South Locality Hub and offer this to clinical staff with good IT knowledge and skills, that are keen to work in a busy and friendly single point in Wymondham. Ideally you will have community or primary care nursing experience although there will be training and support offered. Up to 20% of your time will be practically assisting with community patients in areas of your competence when need occurs, the remaining 80% of your time will be working alongside other triage clinicians being responsible for the smooth triaging of referrals to the south locality nursing hub., Key Areas of Responsibility
- To process referrals by evaluating the information received and allocating to the appropriate service indicating the priority response necessary.
- To document triage notes in order that the patient's record is complete and for audit purposes.
- To use information technology to process the referrals for the 80% triaging role.
- To liaise and communicate with the general public, other health care professionals, and multi-agencies on a daily basis.
- To assess, prescribe and deliver care packages for patients, for example with long term conditions, palliative care and rehabilitation needs, to achieve quality of life and independence where possible.
- To work within the integrated team to facilitate early discharge from hospital.
- To work within the integrated team to prevent unnecessary admission to hospital.
- To work with all health care professionals, and statutory/non statutory agencies to provide a seamless, integrated service to our service users.
You will need to display excellent communication skills, problem solving and documentation with attention to detail and the ability to manage challenging and changing situations.
