Full Job Description

The Norwich Triage team currently has a vacancy for a Community/Urgent Community Response Triage Nurse. The Triage Hub is the starting point of the patient journey into community services, we take great pride in our work and always aim to do the best for our patients. We are looking for someone who holds the same values and wants to be part of a diverse and supportive team. We are a small and friendly team of nurses who triage patient referrals from across the Norwich area. The Community Triage Nurse is a pivotal role within the Triage Hub and is essential in ensuring patient flow across the service. You should be competent and have evidence-based knowledge of a wide array of nursing interventions to be able to triage effectively. Supporting and coaching patients via telephone is also part of the role, therefore proficient telephone and IT skills are important. Although experience of Triage would be an advantage it is not essential as full training will be provided. The role allows for development of enhanced assessment skills with clinical judgement being at the forefront.   • To process referrals by evaluating the information received and allocating to the appropriate service indicating the priority response necessary.
  • To document triage notes in order that the patient's record is complete and for audit purposes.
  • To use information technology to process the referrals for the 80% triaging role.
  • To liaise and communicate with the general public, other health care professionals, and multi-agencies on a daily basis.
  • To assess, prescribe and deliver care packages for patients, for example with long term conditions, palliative care and rehabilitation needs, to achieve quality of life and independence where possible.
  • To work within the integrated team to facilitate early discharge from hospital.
  • To work within the integrated team to prevent unnecessary admission to hospital.
  • To work with all health care professionals, and statutory/non statutory agencies to provide a seamless, integrated service to our service users
