Full Job Description

We are looking for an enthusiastic and highly skilled NMC Registered Complex Care Nurse to join our Integrated Complex Assessment Team on a 6 month fixed term basis.
Our team is responsible for the assessment and tracking of patients requiring a Discharge to Asses (D2A) bed based service within Stoke-on-Trent, Staffordshire and surrounding areas.
The Complex Assessment Nurse will manage a caseload of patients within the Discharge to Assess setting and be responsible for the completion of Continuing Healthcare assessments within a timely manner relieving the pressures to the acute hospitals.
The role will involve close working with Health and Social Care staff and an understanding of Community Home First Services alongside knowledge of the Care Act 2014 and the principles of Discharge to Assess (D2A) would be desirable.
Due to the nature of this role you must be able to adequately travel over a large geographical area therefore a valid driving licence access to a car is essential.
Typically shifts are 8am-4pm Monday to Friday., You will facilitate and develop individual programmes of care and care packages, where necessary, providing specialist information and support to patients, their relatives and other members of the caring team to ensure continuity of a high standard of care.
As part of this role you will also be expected to:
+ Provide clinical expertise and knowledge, demonstrating a sound understanding of the issues related to the identification, assessment, treatment and management of the client group and demonstrate the ability to work autonomously and manage own caseload.
+ Provide specialist nursing advice to the multi-professional team, taking into account all aspects of Clinical Governance promote and develop the use of evidence based practice to improve the outcomes of patient care.
+ Monitor and improve standards of care through, supervision of practice, clinical audit, evidence-based practice, teaching and supporting professional colleagues.
+ Promote involvement and engagement of service users and their families, in the assessment, planning, delivery and evaluation of care.
+ Support the health care team to provide a suitable learning environment that is conducive to the acquisition of further skills and knowledge, for formal and informal personal and professional development.
+ Provide guidance and support to junior members of staff and comply with the NMC standards for mentors
