Full Job Description

Are you motivated and enthusiastic Nurse or Occupational Therapist who wants to make a real difference? We are looking for Complex Discharge Facilitators to join our Inpatient Pathway Team following permanent funding agreement for these posts. The team works across both Herefordshire and Worcestershire to ensure our patients can be discharged from inpatient settings in a timely manner. We would welcome applications from staff who have experience of working in adult and older adult mental health services.

This role is 37.5 hours however 18.75 hours + will be considered. The working pattern is between Monday Friday with service hours primarily being between 09:00 to 17:00, The trust offers flexible working opportunities. You will be based at one of the Inpatient bases in either Worcester, Bromsgrove or Hereford, you will be expected to travel to other sites however expenses will be covered by the trust expenses policy., You will be joining the service at a time of rapid development allowing you the opportunity to support the implementation of new pathway initiatives and ways of working. You will work collaboratively with inpatient wards and partner organisations including the local authority.

You will be expected to provide clinical expertise regarding admission and discharge criteria. To be a part of the multi-disciplinary team, influencing responsive and timely inpatient admissions where length of stay is proportionate to need.

The post holder will be expected to utilise their clinical and risk assessment skills to ensure the best outcomes for patients as part of the admission and discharge process. Effective communication and sharing of information will be key to the role.
