Full Job Description
To work closely with ward staff involved in discharge planning to ensure that the assessments andreferrals for services relating to the patients discharge is completed comprehensively and in a timely manner.Thiswillinvolvepatientfacingcontact
Thepostholderwillundertaketheroleofidentifyingcomplexdischargesandfacilitating assessments for the complex discharge of patients to community serviceswhichwillensure theirsafedischarge fromacutecare.
· To work closely with ward staff involved in discharge planning to ensure that the assessments and referrals for services relating to the patient's discharge are completed comprehensively and in a timely manner.
· To ensure the delivery and maintenance of a high standard ofcarethatispatientcentredandevidencebased.
· To work with teams and specialties across the system to facilitate safe discharges in a timely manner.
· Adhere to the NMC Code of Professional Conduct, Guidelines and Standards and ensure colleagues do too.
· Provide high standards of evidence-based patient centred care and knowledge for all patients,respecting dignityandprivacy.
· Deliver high quality care for patients within the discharge process and ensure all discharge plans are robust toprovide aseamlessandcohesivetransitiontoothersettings.
