Full Job Description

This post holder will work as part of the integrated discharge service, with supervision from the team leader for patient flow to support and co-ordinate discharges from the hospital site. The post Holder will be based at North Manchester General Hospital working as part of a multidisciplinary and multi-agency team and in collaboration with other services including the on site flow management team, hospital staff and community teams.

The post holder will help facilitate timely referrals and support appropriate discharges from hospital. When required will undertake assessments, care planning and integrated working responsive to patient and careers needs.
