Full Job Description
We are looking for someone to join our friendly, supportive team to work with and develop the Peri-Operative service based at Worthing Hospital. This has the possibility to be a hybrid position encompassing both day to day care of patients living with frailty within the Department of Medicine for the Elderly as well as working for the Surgical Department in ensuring better outcomes for those who require surgery.,
- Deliver local preoperative assessment and optimisation to older and frail elective surgical patients in the outpatient setting.
- Ensure 'fitness' for surgery and thus reduce late cancellations for surgery.
- Proactively identify in-patients requiring POPS input on the wards and provide timely and evidence based in-patient assessment, optimisation and coordinated management of care.
- Collaboration with current pathways including enhanced recovery programmes.
- Provide standardised, safe & effective care for older people undergoing elective and emergency surgery.
- Reduce incidence and severity of post-operative medical complications in surgical patients.
- Identify and treat post-operative medical complications early and thus reduce the impact on functional status and cognitive status.
- Utilise community services more effectively (establish relationships with intermediate care to facilitate early discharge).
- Reduce length of stay for surgical inpatients.
- Reduce readmissions.
- Provide education to the wider MDT as part of the Divisions of Medicine & Surgery educational program
- There is responsibility for general professional training of the junior members working alongside POPS colleagues and contributing to the training of nurses and other staff and participating in the activities of the Postgraduate Medical centre including clinical audit
