We are excited to be recruiting Consultant Practitioners in Frailty to work alongside our existing Consultant Workforce at Urgent Community Response and Frailty H@H Virtual Ward. The Team operates across Mid and North Hampshire to deliver acute hospital level care at home in response to urgent changes in health status for patients living with frailty. These vacancies are a direct result of our success and the recent expansion of the service through increased commissioning and a very successful GIRFT review. The successful applicants will be joining an established and supportive group of Consultant Practitioners from varied backgrounds., Due to the expansion of the Hospital at home frailty Virtual Ward (H@H), we are seeking a consultant practitioner to work alongside the existing consultant practitioner and foundation consultant practitioner to provide senior clinical oversight seven days a week. The Urgent Community Response Team (UCR) is a rapid assessment, diagnostic and treatment service to reduce unscheduled admissions for people with complex health needs, exacerbation of long-term conditions and frailty. Patients referred to the service are triaged as to be seen 'within 2 hours', or 'same day' for an urgent assessment. The H@H Frailty Virtual Ward supports patients who would otherwise be in hospital to receive the acute care and treatment in their own home, which prevents them from deterioration that would require an admission. The UCR and Frailty Hospital at Home Virtual Ward team operates from multiple bases across Mid and North Hampshire. The service operates 0800hrs-2000 7 days a week, 365 days a year., Further information about the Trust and this role can be found on the Job Description and Person Specification document attached. - Professional UK registration NMC / HCPC with evidence of sufficient experience to meet the requirements of the Job Description
- Advanced Clinical Practice Master's degree OR a health related master's degree AND evidence of equivalence of Advanced Level practice capabilities via the portfolio route.
- Independent non-medical prescribing or pharmacology level 7 profession relevant qualification
- Management qualification or significant senior management experience in a healthcare setting.
- Evidence of significant professional and managerial development, including a comprehensive and relevant CPD portfolio.
- Evidence of working at academic level 8 and or through competency-based assessment of application of level 8 working.
- Teaching Experience / Qualification
- Able to demonstrate a good command of the English Language both written and verbal
- Computer Literate in both data entry and communicating via IT - emails and reports
