Full Job Description
The Home Treatment Team has the capacity to take on approximately 25 patients at any one time. Crises Resolution Services are provided within the local community mental health services and do not form part of this acute care model. The Acute care Teams work collaboratively work with CMHTs to ensure timely communication and care planning. The care of individuals is subject to the Mental Health Measure (Wales) 2010 and the post holder is expected to work in line with the requirements of the MH Measure. The post holder will provide patient centered recovery based care through a supervisory and consultancy role. They will provide Responsible Clinician oversight for approximately half of the ADULT acute care service covering 10 beds and half the HTT patients alongside the other Substantive Approved Clinician. Main duties of the job The post holder will be the treating consultant alongside the other Approved Clinician for inpatients admitted to Dinas Male and female wards and those accepted for Home Treatment Team. The adult inpatient wards admit patients from Denbighshire and Conwy and occasionally from the East and West areas. The Division has an active bed management process, which allows flexibility in managing the available beds. On occasion patients are admitted to the corresponding units in the east and west localities. Admissions to the wards typically come from the community via CMHT or the general hospitals via Psychiatric Liaison Service. Medical oversight of admissions occurs with a handover of consultant responsibility at the time of admission and discharge. The post holder and colleagues in the community work closely to facilitate continuity of care and discharge planning. The CMHT remain involved in patient care when admitted. The post holder will be the responsible clinician for patients admitted under the Mental Health Act. They will provide medical leadership to the multidisciplinary team. The post holder will work closely with the sector consultant, CMHT and local authorities to provide continuity of care and robust discharge planning. The post holder will work using the Mental Health Measure and local policies. The post holder will be the supervising doctor for junior doctors attached to the post.
