Full Job Description

The Bedfordshire Luton and Milton Keynes (ICB) is responsible for planning and allocating resources to meet the four core purposes of the Integrated Care System (ICS), namely: to improve outcomes in population health and healthcare; tackle inequalities in outcomes, experience, and access; enhance productivity and value for money; and help the NHS support broader social and economic development.
Within this context, as the ICB develops as a statutory organisation, a significant challenge will be to determine how it effectively supports and enables place-based and local partnerships to take necessary action that will improve population health outcomes and reduce inequalities.
If you need support with the application process, please contact the ICB [email protected], The post holder will work as part of an integrated ICB team, alongside system colleagues plus regional and national teams to help deliver the best value from Local and National programmes and services by providing clinical expertise to support The Continuing Health Care's integration pilots and projects and to embed new clinical services to fully realise their potential.

This opportunity is for a Zero hours contract and will form part of our Bank staffing program.
Training will be given for candidates that meet the the specifications outlined., To manage a case load of patients through the Continuing Healthcare process, utilising knowledge, and experience to complete holistic assessments and reviews both remotely and in person. Supporting our partners and the public with insight and application of the National Framework.
