Full Job Description
An exciting opportunity has arisen within the Sheffield GP Collaborative.
The Sheffield GP Collaborative (GPC) provides an urgent out of hour's primary care service for the city of Sheffield. This includes providing clinical triage for patients referred by NHS 111, face to face clinical assessment at the GP collaborative and when required home visits for the housebound and those at the end of life.
The purpose of the role is to provide a high-quality service for patients and healthcare professional using the Out of Hours GP Service. To achieve and maintain high standards, developing effective working relationships with all service users. To provide support to GPs, Advanced Care practitioners, colleagues, patients and other health professionals as required.
The role involves working unsociable hours including overnight shifts, weekends and bank holidays.
Main duties of the job
To receive incoming telephone calls from patients and clinicians in a courteous, confident, efficient and confidential manner.
Deal with telephone queries giving advice, transferring calls or taking messages as appropriate to the call.
To support the triage clinicians ensuring patient demographics are completed on all patient referrals in a legible and concise manner.
Assist with Health Information enquires from the general public and Health Care.
Process calls transferred electronically from NHS 111 and other health care providers.
