Are you a highly organised and proactive individual with a passion for supporting healthcare services? We are looking for a dedicated Coordination Support Administrator to join our dynamic Urgent Community Response (UCR) Team for a 6-month fixed term / secondment to support a pilot approach to care coordination. This vital role is key to the smooth and efficient delivery of urgent community care, enabling patients to receive timely interventions that prevent hospital admissions and support recovery at home. You will be at the core of our fast-paced service, coordinating patient visits and ensuring that our healthcare professionals can respond to urgent referrals with speed and precision.
As part of the UCR Team, you will play a crucial role in supporting the delivery of high-quality care, acting as the central point of contact for clinicians, patients, and external partners. This is a fantastic opportunity to be part of an innovative service that is making a real impact on patient care, improving outcomes, and reducing strain on acute hospital services., + Act as a key point of contact for the UCR service, managing urgent referrals, coordinating communication between clinicians, patients, and external services.
+ Provide comprehensive administrative support, including diary management, scheduling patient visits, and maintaining accurate records of patient care.
+ Prioritise and coordinate rapid response team activity, ensuring urgent cases are actioned swiftly and efficiently.
+ Use electronic patient record systems to track and update patient information, referrals, and outcomes.
+ Liaise with GPs, community services, and other healthcare professionals to ensure smooth communication and continuity of care.
+ Assist in the collection and reporting of data for service audits, performance monitoring, and quality improvement., + The healthcare provider of first choice for the people of East Sussex.
