Full Job Description

Gloucestershire Health and Care NHS Foundation Trust (GHC) and Integrated Care-24 (IC24) are pleased to share that they will soon be providing the Integrated Urgent Care Service (IUCS) for Gloucestershire including Out of Hours Treatment and GP Home Visiting and are looking for service Coordinators to join GHC on bank contracts. The GP out of hours service will be accessed through the NHS 111 service and the Clinical Assessment Service. Coordinators will be operational outside of the period of 9-5 Monday to Friday. We are looking for highly motivated individuals with good working knowledge of health services to oversee service activity, reporting and escalation of IUCS delivery for all bases within the county and are responsible for booking home visits following a triage. The Coordinator is the main point of contact Out of Hours and are a conduit to the in-hours management team., Oversee the operation of the out-of-hours integrated urgent care service, ensuring that all activities are carried out efficiently and in accordance with established protocols. Be responsible for booking GP home visits via SystmOne case management and car dispatching.
