Full Job Description
The Fenland Adult Locality Team, based in Wisbech, works with adults in the Fenland area who suffer with severe and enduring mental health. The role will involve holding a caseload of patients with differing mental health conditions.
Your role will support patients to move through their recovery journey. As a CPN, you will be part of the nursing team, including administering depots and holding patients on Clozapine. You will work autonomously whilst working as a fully functioning MDT.
As the smallest locality team in the Trust, we pride ourselves on having a healthy work environment, where you will be encouraged to deliver high quality practice, and you will be fully supported to continue your CPD to enhance your practice and skills., To take on the role of care coordinator for clients and provide leadership to support others, to ensure that full assessment of individual care needs are undertaken, that care is planned, implemented and reviewed on a regular basis.
To participate on the duty rota.
To act as a role model and promote effective teamwork across the service to meet individual clients needs.
To ensure that the Mental Health Act is adhered to at all times, and to work within the boundaries set out by the Trust policies and procedures., The post holder will hold a case load of patients suffering with severe and ensuring mental illness, working towards the recovery model to support patients to achieve positive outcomes.
The post holder will provide a good role model, develop and maintain good systems of communication, be able to communicate with a wide range of people in a range of situations.
Reporting verbally and in writing with regards to clients progress, assessments and care planning to the wider multi-disciplinary team.
Maintain accurate and complete records.
Discussing with client and their families / carers regarding care planning, assessments and treatment.
Be able to acknowledge the barriers to effective communication and adapt communication style in response.
Maintain professional standards in line with NMC code of conduct and Trust Nursing Strategy.
The post holder will monitor and maintain the health and security of self and others.
Managing identified risks.
Ensuring that health and safety legislation is adhered to.
Manage emerging situations, taking action as appropriate.
Reporting actual and potential incidents.
Attend Trust mandatory training.
The post holder will contribute to the continuous improvement of services.
Identifying the implications of direction, policy and strategy on current practice in a constructive and timely manner.
Actively involve service users and the public in service improvements.
Timely investigations and response made when dealing with PALs issues in line with Trust policy.
