Full Job Description

The Derbyshire Recovery Partnership are looking to recruit a Substance Use Practitioner for a fantastic opportunity in Chesterfield (office located on the outskirts of town in S42 7) working intensively with service users presenting with drug and alcohol issues, are involved in the criminal justice system, and who may be in prison or in the community. The role will involve co-ordinating care and delivering psychosocial and harm reduction interventions to service users in structured 1:1 key work and group work sessions. This cohort of people accessing our service may be subject to licence conditions, Alcohol Treatment Requirement (ATR) and Drug Rehabilitation Requirement (DRR) court orders, and may be involved with the Integrated Offender Management (IOM) team, Courts, Liaison and Diversion, Prison healthcare, Probation services. The focus of this role is helping engage service users in a care planned and recovery-focussed way to attain improved health outcomes, achieve their personalised treatment goals and reduce criminality. This role involves managing a significantly reduced caseload where you will provide a high intensity and frequency of support to people living in the Chesterfield community or who may be in prison and preparing for release to the Chesterfield area. As we conduct prison in-reach primarily in HMP Foston Hall, HMP Nottingham and HMP Ranby, part of your role will involve conducting prison in-reach; assessing the treatment needs of people leaving prison and supporting them by involving other services as you plan comprehensive community-based care. You will liaise closely and regularly with other relevant professionals and involve specialist services within your care coordination role. This role will be based in Chesterfield in Derbyshire, working 37.5 hours per week; Monday-Friday 9am-5pm, on a permanent contract. Due to the outreach and responsive nature of this role, applicants must have a driving licence and access to their own vehicle for work. Travel expenses for outreach is reimbursed and there is staff car parking is available to the rear of the office. Successful applicants will be able to demonstrate a clear passion for working with people using drugs and/or alcohol that are also involved in the criminal justice system. Applicants will have an innovative, motivating and proactive approach to work and experience of using appropriate models, interventions and approaches tailored to meet the individual needs of people accessing our service with varying and complex needs.
