Full Job Description

To work as a Team member in providing a Crisis Resolution Home Treatment Service (CR/HT) in North Powys. The CR/HT provides a service for adults who are experiencing a Mental Health crisis and are at risk of admission to Hospital. Intensive Home Treatment will be provided in collaboration with the service user and in the least restrictive setting, 7 days a week, and 365 days a year as per rota. CR/HT will also facilitate the early discharge of those admitted to inpatient units, through intensive but time limited intervention in the community
We provide a service to people in acute mental health need, and offer an alternative to going into Hospital by providing intense support at home. We work 9-9, 7 days a week every day of the year, so shift-work is essential in a mixture of 12-hour and 8-hour shifts.
We are looking for a qualified practitioner- a Nurse, Social Worker or Occupational Therapist. You will need to have experience of community mental health work and to be passionate about this area of work. We are a supportive team, with a low turnover of staff and we work in a beautiful rural area.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
