This Crisis Response Team and Hospital at Home Senior Discharge Coordinator, Health and Social Care Practitioner role will work in the community with Crisis Response Team (part of Manchester Community Response).
The Crisis Response Team and Hospital at Home Senior Discharge Coordinator, Health and Social Care Practitioner is responsible for junior members of staff, undertaking informal and formal support and supervision.
The Crisis Response Team and Hospital at Home Senior Discharge Coordinator, Health and Social Care Practitioner will be required to actively participate in new initiatives and further changes intended to improve service delivery.
We are developing a hospital at home, resourcing the team to enhance our established "Home First" approach supports patient's personalchoices, strengths, and ensures inclusion in every stage of their care.
We are developing the service to adopt earlier intervention from our community response teams, which aims to reduce acquired hospital deconditioning, enabling a person to maintain as much of their independence as possible.
We are passionate about continually developing our services to provide gold standard care for older people., This Crisis Response Team and Hospital at Home Senior Discharge Coordinator, Health and Social Care Practitioner role will work in the community based with Crisis Response Team (part of Manchester Community Response),assisting registered professionals (Advanced Clinical Practitioners, Physiotherapists, Occupational Therapists, Nurses, Pharmacists and SocialWorkers).
As part of the multidisciplinary team, the post holder will be responsible for supporting the Crisis Response Team to deliver urgenthealth and social care to people in crisis and at risk of hospitalisation. They will also support the Hospital at Home service to reduce length of stay in hospital and provide care for patients within their own home.
The successful candidate will be responsible for junior members of staff, undertaking informal and formal support and supervision. The post holderwill be required to actively participate in new initiatives and further changes intended to improve service delivery.
