Full Job Description
You will work within the First Coastal Primary Care Network. This role is part of a new place-based mental health workforce that is integrated and co-located within Primary Care Networks. You will be required to work autonomously within the context of local and national drivers, providing advanced clinical care and liaison into Primary Care Networks, using high level assessment to determine presentation, treatments, refer appropriately and co-ordinate the child/young person's journey across the health, social care and voluntary sector system, regardless of care setting., Key aspects of the role include: + To work as an accountable and autonomous practitioner working within specified GP practices to provide specialist children and young people's mental health input within the primary care setting. + To work with a caseload of children and young people referred to them in a manner which demonstrates an enhanced knowledge base and advanced clinical skills and competencies. + Establishing effective working relationships with children and young people, their families and carers, including education settings, and promoting individual rights, recognising and respecting their contributions to care planning, placing the child/young person at the centre of their care.
This is one of three posts that are working with three PCN's. Base is negotiable between PCN, LPFT and successful candidate. You will be required to travel around this large rural county.
