We discourage over reliance on AI-generated as it will lessen your chances of success in securing an interview with us ., The post holder will work in collaboration with team colleagues to deliver safe, efficient, high quality and effective services by providing strong clinical leadership and ensuring alignment of team based priorities with the Directorate and Trust priorities and values.
The post holder will:
+ Independently manage a specialist clinical caseload with support and supervision provided by senior staff via various methods e.g. face to face contact, email, telephone. This will include provide clinical assessment and the delivery of treatment programmes for children and young people with a diverse range of physical healthcare needs
+ Consolidate undergraduate clinical experience, develop independent clinical practice, participating in continuing professional development
+ Work within a variety of setting such as community, homes, schools, nurseries, hospital ward & designated Clinics. The service provides a blended offer of delivery including Face2Face and a digital offer (Attend Anywhere).
+ Adopt corporate, clinical governance and best practice principles within service delivery
+ Demonstrating a commitment that the "voice" and lived experience of the child and young person is at the heart of all CYPS decision making and clinical care planning
A valid Driving Licence is essential for this post.
We welcome applications from newly qualified nurses, As a Band 5, the post holder will work under the close supervision of a clinical supervisor as well as be supported by their designated line manager
+ Provide a range of health care based assessment and treatment interventions for children and young people (0-18yrs) and their families/carers within the designated team. Provision will be in accordance to the application of a range of profession specific models of clinical practice. This will include:
+ Providing a holistic clinical/health assessment and delivery of intervention programmes for children and young people with a range of needs. Care planning will utilise a range of treatments, skills and options for use in both the home, community and school setting.
o A personalised and coproduction approach will be offered when working with families, parents and carers
o This includes consideration and identification of safeguarding needs and taking further action where required
The qualification, training & experience requirements for the role are underlined in the Job Description/Person Specification., Contribute to team based, multi disciplinary and multi agency based discussions
including safeguarding discussions. This includes offering professional challenge to
ensure the welfare and needs of the child remain at the heart of all decision making.
Contribute to the robust implementation of team based routine outcome measures.
Demonstrate awareness regarding assessing capacity and consent with children and
young people. Where such capacity is lacking/absent, to know how to work within a legal framework in the management of clinical delivery.
Support the teams compliance around contractual Key Performance Indicators (KPI's)
and the most effective & impactful use of resources.
Support how the team meets its statutory SEND duty to work with partners across
education, health and social care to better support children, young people and their
families to achieve their goals and outcomes. This includes attending ECHP meetings as
requested
We provide a Multi-professional preceptorship programme that is aligned to the National Preceptorship Framework for Nursing and have been awarded the Interim quality mark from NHS England. Our programme is provided to all Newly Qualified; Nurses, Nursing Associates, Internationally Educated Practitioners, Allied Health Professionals and Return to Practice Practitioners. In addition to the programme we also offer all preceptees the opportunity of enrolling and completing the level 7 (Masters) 15 credit validated module "Preceptorship for Healthcare" (AP7026)., For more information please see the attached 'Additional Information for Applicants' document.
Salary Update
With effect from 1 April 2024, there is a 5.5 per cent consolidated uplift for all Agenda for Change (AfC) staff on NHS terms and conditions. This may change the salary that is advertised on this advert.
