Full Job Description

An opportunity has arisen for a Darzi Fellow +1 year as a way of consolidating learning from the programme and driving forward a project started during the Darzi year. The post holder will be working with the SW London ICS and community partners to implement and evaluate frailty pathways for patients attending St George's, Epsom and St Helier University Hospitals and Health Group. The development of these pathways is a strategic clinical priority for the organisation and year 1 of the Darzi fellowship delivered initial work to establish frailty pathways in line with national best practices, working in collaboration with community providers. This fellow will build on this initial work, working alongside the Managing Director for Integrated Care, Programme Director and site leads, but also working with clinicians to establish new clinical pathways that improve patient experience and clinical outcomes.
Please note this position is only available for internal staff within Epsom, St Helier and St Georges., A key focus for this year 2 Darzi fellow opportunity will be the management of patients with frailty, including work to explore workforce models for community support of frailty, and with AHPs to lead work on rehab and re-ablement pathways. A core requirement will be engagement with stakeholders from across the Trust and community and evaluating evidence based ways to improve care through innovative models.
