Full Job Description

The post holder will be responsible for providing efficient and high-quality clerical support to the Hillingdon Cardiac/Heart Failure Team with particular emphasis on administration support for the NHS National Audit for Cardiac Rehabilitation (NACR)., To use, understand, and develop systems and datasets to meet data submission requirements, ensuring quality assurance of data processes.
To be responsible for collecting, entering and reporting data for national, regional and local purposes.
To work with clinical and administrative teams to update and audit data on the designated databases (or other data collection systems) including S1 and NACR.
To collate and present activity data for service and management meetings and for monthly and ad hoc statistical returns via Tableau or other.
To undertake data cleaning exercises, ensuring data is high quality, properly curated, and robust
To ensure all data items are collated in a timely manner to comply with Trust requirements for uploading and reporting.
To have a comprehensive understanding of computer software such as MS Office and to be the lead on using SystmOne
To establish good communication networks with external stakeholders including local and regional NHS teams as required
Communicating and highlighting to services and data teams of any missing or incorrect data entry.
Communicating project and data information to the target audience in a comprehensive and understandable format.
Contacting GP practices and other hospitals both in and out of the borough for patient information as needed.
Maintain service line databases for record keeping, ensuring accurate reporting.
Complete regular archiving of patient notes in line with trust policies and procedures.

The post holder will play an integral part of the team to meet additional data collection and data reporting requests. The post holder will monitor, input and collate data, with a focus on ensuring efficient, secure and sustainable use of data and information management processes and systems into appropriate databases. The post holder will support development of data collection tools/ databases to meet the requirements of national, regional and local data requests. The role will require high attention to detail, ensuring data collection and data input is of high quality and accurate.
The post holder will have close and collaborative working relationships with the Team Lead, wider clinical staff, and other administrators, as well as external agencies.
The post holder will have excellent interpersonal skills and have the ability to communicate effectively with service users, carers, health professionals and team members.
The post holder will work under own initiative with a flexible and adaptable approach to the job role in order to meet the changing service needs.
