Full Job Description

As part of our service expansion, we are excited to announce a new opportunity: Safe Systems, Pathways, and Transition Lead. This role will be integral to our Mental Health, Learning Disabilities, and Complex Needs service area, overseeing and coordinating the transition process for young people moving from children's to adult services. As the Safe Systems, Pathways, and Transition Lead, you will play a key role in ensuring that transitions from child to adult services are seamless and effective. This will involve providing leadership and management support to social workers across both children's and adult services, ensuring comprehensive planning and implementation of support systems. You'll work closely with individuals, families, NHS Trusts, service providers, and community organisations to ensure every person receives the support they need through every step of their transition to adult services. Additionally, you will take a lead in supporting safe discharges from hospitals within mental health and learning disability services, ensuring that appropriate legal frameworks are in place and statutory requirements are met.,

  • Lead transition planning from children's to adult services for individuals with learning disabilities and mental health needs.
  • Provide supervision, guidance, and training to social workers and their managers across both children's and adult services to ensure effective and coordinated transition planning.
  • Collaborate with senior managers in learning disabilities and mental health services to align transition planning and resource allocation with NICE guidelines.
  • Ensure legal frameworks are in place and followed to support safe hospital discharge and transition planning.
