Full Job Description

You'll be joining a passionate team of dedicated transition workers, enablement staff and social workers who work alongside people with learning disabilities and their families throughout their lives.

You'll work with individuals and carers to maximise independence and social inclusion through strengths-based interventions, working closely with health professionals and partner agencies to provide personalised, empowering support.
