An exciting opportunity for an experienced Dementia and Delirium Clinical Nurse Specialist has arisen to join our Corporate Nursing Directorate. In this role, you'll be an integral part of a dedicated team, working together to enhance patient care and outcomes for people living with dementia and delirium. We're looking for someone with a genuine passion for elderly care and a proven ability to drive quality improvements. You'll be working closely with the Dementia and Delirium team, where your expertise will help shape better patient experiences. Excellent influencing and negotiation skills will be key, as well as the ability to challenge when required and advocate for best practices.   • The post holder will champion the rights of patients living with dementia and delirium in hospital, ensuring they are treated with dignity and respect and that care is person-centred.
  • The post holder will support the development, implementation and monitoring of best practice standards in the care of people living with dementia and delirium, and their carers when they are admitted to hospital.
  • The post holder will support the comprehensive screening and assessment of patients 65 years old and over to identify people with cognitive impairment and delirium and appropriately refer to enable access to support services.
  • The post holder will participate in the provision of effective education and training in all aspects of dementia and delirium care.
  • The post holder will support the Lead Dementia and Delirium Practitioner to ensure that all patient pathways are followed, audits are undertaken, screenings are performed and education and training is provided to all staff within the organisation.
