Full Job Description
Dementia UK & Derbyshire Community Health Services NHS Foundation Trust are working in collaboration to provide a new and exciting Admiral Nurse post within the existing Derbyshire Dementia Palliative Care Team. This post will be covering the south of the county with a base at the Florence Nightingale Community Hospital in Derby.
Admiral Nurses are specialist dementia nurses who offer expert practical, clinical & emotional support to families involved in the care of a person living with dementia with complex needs. The Admiral Nurses will enable improved palliative care for people living with dementia and their families, working with professionals within the Dementia Palliative Care Team, & wider MDT.
We are looking for exceptionally skilled passionate Registered Nurse with skill & experience in providing person-centred and relationship-centred dementia care. Combining direct work with families with supporting the best practice of health and social care professionals, you will call upon your impressive knowledge of bio-psychosocial interventions to improve the experience and quality of life of families affected by dementia., The Admiral Nurse will support, deliver and coordinate relationship-centred dementia care within The Dementia Palliative Care Team, Derbyshire Community Health Services NHS Foundation Trust, to improve the quality and experience of palliative and end of life care for families affected by dementia.
In line with the Admiral Nurse Competency framework at a specialist level, the post holder will provide:
o Specialist bio-psychosocial assessment and evidence-based palliative and end of life care for people with advanced dementia and their families
o Support education, training and best practice for staff in the delivery of evidence-based dementia care
o Support with quality improvements in relation to palliative and end of life dementia care
o Consultancy and guidance to other health and social care professionals regarding palliative and end of life dementia care, to increase knowledge, competence and confidence
