Our Dementia Support Team working in North Norfolk and Norwich have a rare opportunity for a Band 5 Community Mental Health Nurse to join their established team.
We are looking for colleagues who share our compassion, drive and belief that people can live fulfilling lives with dementia.
Our dedicated community team work with people following a diagnosis with dementia, optimising their treatment and making sure they are able to access the help they need.

If you're looking for a new challenge, in a rewarding, collaborative service - we would love to hear from you!, Our Memory Assessment and Treatment Team is based in Norwich and covers the city and much of North Norfolk.
Our Dementia Support Nurses monitor the titration of medications that can enhance memory and cognition as well as signposting people to other services they might need.
The work involves helping patients, families and carers with a recent diagnosis of dementia with post-diagnostic support, monitoring treatment (including the monitoring of medication), Cognitive Stimulation Therapy and groups. Staff often work independently, visiting patients in the community or using remote methods, However, everyone is also part of a very supportive multi-disciplinary team where they can consult each other to ensure we are providing the very best care.
The role is focused on people who have had a recent or established diagnosis of dementia and will involve working with patients, families and carers in group work, individually, remotely and face-to-face. The Trust has now introduced a DBS Update Service which is a contractual requirement. You need to subscribe when you have applied for your DBS clearance and there is a time limit to subscribe of 19 days after receipt of your DBS disclosure. Please ensure that this is completed within the set time scale.
This update service is an annual subscription at a cost of £13 to you. This will enable the Trust to have instant online access to your DBS record, with your consent, and so will remove the need for you to have to apply for this repeat check again.
