Full Job Description

As a member of our Team your role will include direct care delivery within the Patient's own homes. This role will be primarily working within the new Blood Transfusion pathway and so will require a innovative individual used to working in a flexible manner.

An exciting opportunity has arisen to join our UCLH@HOME team as we pilot a new Blood Transfusion pathway. We are forefront of integrated care - providing innovative and transformational care that best serves local communities. UCLH@Home is an NHS service that helps by supporting patients to leave hospital once medically optimised and safe to be managed in the community.
