Full Job Description

An exciting opportunity has arisen for a Band 6 Deputy Lead in Dementia Care Mapping based within Mental Health Services for Older People (MHSOP), covering Cardiff and Vale. This initiative is partially funded by the Regional Partnership Board. The Deputy lead will be an advocator and skilled practitioner in Dementia Care Mapping and support the Clinical lead with the continued service development and mapping coordination across Cardiff and Vale. You will be willing to undertake the DCM - Leading Practice Change qualification.

The DCM service supports and coordinates mapping activity across Cardiff and the Vale of Glamorgan through individual referrals, rolling programmes of cultural mapping cycles, evaluations and also with the possibility of being involved in future research projects., The post holder will continue to support and develop Dementia Care Mapping as an approach to improve person centred care within health, social and third sectors across Cardiff and the Vale of Glamorgan. The Dementia Care Mapping team works alongside and extensively collaborates with, the Dementia Learning and Development Team to promote and embed improved practice for the support of individuals living with dementia.

As deputy clinical lead you will undertake DCM as a key focus in your role, as well as supporting and co-ordinating mapping that is undertaken across the services, providing supervision, delivering the strategy for DCM practice, whilst ensuring governance. You will support the monitoring activity data and undertaking service audits and evaluations.

You will contribute to the ongoing professional developmental needs of DCM trained staff in line with the professional standards set out by Bradford University DCM guidelines, the Good Work Framework and for developing a service that meets Standard 16 of the All Wales Dementia Care Pathway of Standards.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
